You are eligible to enroll in USD 231 benefits if you are working 20 or more hours per week. Employees working 20-30 hours per week may enroll for benefits and pay for the full cost of the products.

• Support staff - eligible to enroll the first of the month following one month of employment.

• Certified staff - eligible to enroll the first of the month following the date of hire.

Employees working 30 or more hours per week will receive the USD 231 contribution towards the cost of medical and dental benefits, provided the employee enrolls in health and dental coverage. If the employee does not elect USD 231 health or dental coverage, the paid benefits will not be applicable.

Eligible dependents include your legal spouse and your dependent children to age 26.

Coverage for dependent children terminates as follows:

• Medical, Dental, & Vision – end of the year in which they turn 26

• Life Insurance – end of the month in which they turn 26
